Insurance: Our office participates in many insurance
plans. As a courtesy to our patients we will file insure
forms. Some plans require referrals from your primary
care physician for podiatric care. It is the patient's
responsibility to acquire these referrals. Please be
aware that you are responsible for your bill if you
do not obtain the proper referral. All co-pays for HMO
and PPO type insurance plans are due at the time of
your visit.
Medicare/Medicaid: We participate in Medicare and Medicaid.
Medicare patients will be required to pay the 20% co-pay
at the time of their visit. Our office will file your
claim promptly
