INSURANCE PLANS

Our office participates in many insurance plans. As a courtesy to our patients we will file insure forms. Some plans require referrals from your primary care physician for podiatric care. It is the patient’s responsibility to acquire these referrals. Please be aware that you are responsible for your bill if you do not obtain the proper referral. All co-pays for HMO and PPO type insurance plans are due at the time of your visit. Regarding Medicare and Medicaid, we see Medicare patients and only Medicaid children.
